4. Summarize your achievements in your field in the last 5 years (awards, patents, publications, sales figures, cost efficiencies)

5. Describe your current role (what you do) and where you want to be in the next 5 years.

6. Explain what you do NOT want to be known for

7. What is your chosen communication style? Writing/Speaking/Presenting/Speaking & presenting

8. List any and all professional associations (member-associations, online groups, non-profits etc.) and your roles therein

9. Provide a summary paragraph of your background. Highlight important changes and developments in your personal and professional growth.

10. Please point to any published work, any commercials, videos, etc.

11. Are you involved in any community or volunteer work? If so, what?

12. Do you contribute to or are involved in any charity? If so, who and what?

13. Is there any area outside of career work in which you hold a lot of knowledge? If so, what and why?

15. Are there any strong opinions you have on the services you provide? Either about the education within, industry, competition, marketplace, pricing or future? What are they and why?
